Application for New License

Once you have completed requirements you may apply for your license. When you complete the correspondence courses that instructor will send you “Certificates of Completion.” When you complete the Coaching Workshop your course instructor Dr. Lana Ford will send you a “Certificate of Completion” Please attach these documents to your application form.
