A month prior to expiration of your license you may renew it. There are no educational requirements for renewal.
We do ask that you list new techniques or certifications you may have acquired.

You may mail your renewal application and send $50.00 USD for two years to P.O. Box 163 Ridgecrest, NC 28770

You may use your credit or debit card and email your application to pay@fshlb.com or fax it to 888-728-0011. Pay $55.00 by credit or debit card using the pay pal button below.
